Where the contact of record is a management company, that office usually completes the form itself. Which form the lender sends
For a condominium the lender sends Fannie Mae Form 1076, which Freddie Mac issues as Form 476, the Condominium Project Questionnaire. It travels with the addendum, Form 1076A or 476A, covering building safety and deferred maintenance. Many lenders send their own version instead, asking the same underwriting questions in a different order.
Two things that were true until recently and are not now. The short form questionnaires, Fannie Mae Form 1077 and Freddie Mac Form 477, are retired. So is Limited Review: for applications dated on or after 3 August 2026, Full Review is the regime, which means the full questionnaire is answered even on the loans that used to escape it.
The answers that hold a file up are usually the same few: the split between owner occupied and rented units, how much of the budget goes to reserves, whether a single owner holds more than the permitted share of the units, litigation, and any deferred maintenance or special assessment.

Texas requires an association to state its transfer fee schedule on the face of its management certificate, which is a recorded public document. These are the lines recorded for The Shelby Condominium Association.

A lender questionnaire is not the statutory resale package, and the two are routinely confused. The questionnaire is the lender's own form and is generally neither capped nor timed by statute. The resale package that goes to the buyer is both, and in Texas it runs as follows.
