Which form the lender sends

There is no mandatory cooperative questionnaire. The lender sends its own form, which is why two lenders can ask a cooperative the same facts on completely different paper. Fannie Mae's Form 1074, Request For Cooperative Project Information, is an optional instrument some lenders accept, and its instructions name the managing agent as a party who completes it.

The Condominium Project Questionnaire Addendum, Fannie Mae Form 1076A and Freddie Mac Form 476A, does apply here despite its name. Its own first line states it covers both condominium and cooperative projects, and it is the one that asks about building safety and deferred maintenance.

The answers that hold a file up are usually the same few: the split between owner occupied and rented units, how much of the budget goes to reserves, whether a single owner holds more than the permitted share of the units, litigation, and any deferred maintenance or special assessment.

What Virginia sets by statute

A lender questionnaire is not the statutory resale package, and the two are routinely confused. The questionnaire is the lender's own form and is generally neither capped nor timed by statute. The resale package that goes to the buyer is both, and in Virginia it runs as follows.
